Request Counseling

Individual Counseling:
We offer counseling to adults who are suffering and struggling to deal with
complex life situations. Counseling for minors of Salem Heights Church members will be considered on a case-by-case basis. We are happy to make referrals for adolescents outside of our ministry.
Marriage Counseling:
We offer counseling to husbands and wives who find themselves needing wise and compassionate help for complex marital dynamics. At this time, we are only able to offer marriage and pre-marital counseling to Salem Heights Church members. You can check this website occasionally to see if our availability for marriage counseling has changed.
Family Counseling:
We offer counseling to families who could benefit from having an outside voice offer hope and help in the midst of family hardships.
Our counseling ministry offers short-term counseling. We are unable to offer crisis counseling. If you are in crisis and need immediate help, please call 911 or the Psychiatric Crisis Center at 503-585-4949.
A one time $35 resource fee will be required for a first appointment.
Donations for counseling services are always welcome.
Scholarships are also available.

Step 1:
Fill out our online intake form.
Get started by providing us with some initial infomation. Fill out our Intake Form online or download a PDF to print. Please bring to the church office when completed.
Step 2:
Appointment Scheduled
After your intake forms have been received/processed we will get in touch to schedule your first appointment.
Please submit pre-marital applications at least 6 months before your wedding date in order to allow adequate time for the counseling process.
Please note that we are experiencing a high level of individual and marriage counseling. Wait times are increasing. Our office will contact you within one week of receiving your online request to discuss scheduling appointments.
It may take up to 3 months for you to see a counselor depending on your availability and the type of counseling. Salem Heights Church attenders may experience a wait of up to 4 weeks.
A Note on Remote Counseling:
We are able to offer remote counseling on a case-by-case basis to local clients only.
